Location: Depending on program requirements, may require remote or in person program location(s) or hybrid. Requires office-based schedule for up to the first 6 months of hire. Must reside in or near service areas of El Dorado, Placer, Sacramento, Sutter, Yolo, and/or Yuba Counties.
This position requires half of time spent in El Dorado, and the remaining half in Sacramento.
SUMMARY
Under direct supervision of the Supervisor(s), NCM is responsible for the evaluation of the participant as a total person and identifying and evaluating the functional and health limitations impacting participants’ overall health and well-being.
ESSENTIAL DUTIES
1. Conduct in-depth assessments/reassessments covering medical, health, and rehabilitation concerns.
2. Certify level of care determinations.
3. Participate in developing the care plan, implementing services, conducting follow-up, and monitoring of client needs and care plan.
4. Document all care management activities in accordance with professional standards and program requirements.
5. May include carrying a small caseload and managing all caseload related tasks.
6. Perform physical assessments as necessary and interpret clinical findings.
7. Coordinate with physicians and other health professionals as needed.
8. Identify and develop health-related services and support systems for the client.
9. Complete annual satisfaction surveys.
10. Collaborate with Care Managers and Supervisors.
11. Function as part of interdisciplinary team member.
12. Participate in trainings and conferences as appropriate to job duties or required by funding source(s).
13. Other duties as assigned.
QUALIFICATION REQUIREMENTS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The qualifying requirements listed below are representative of the knowledge, skill, judgement, and/or abilities required.
This position requires a valid current license to practice nursing in the State of California, and one year experience, geriatrics preferred. Must successfully clear a Department of Justice background check and not be on the Office of Inspector General’s Exclusion List.

EDUCATION AND/OR EXPERIENCE
- Current Registered Nurse license with one year experience with older adults, people with disabilities, or a home health program.
- Strong communication and organizational skills.
- Ability to work as a functional and collaborative member of an interdisciplinary team.
- Requires a positive attitude, desire, professionalism, and willingness to work with low-income, medically compromised individuals, and an ability to work with participants, peers, community members, and collaborative partners.
- Strong communication skills.
- Proficient technical knowledge with current/prior versions of Microsoft and Apple operating systems, Office applications, Outlook/Exchange, Microsoft SharePoint.
LANGUAGE SKILLS
Fluent in English with good communication and writing skills, bilingual preferred.
MATHEMATICAL SKILLS
Ability to perform simple arithmetic calculations.
REASONING ABILITY
Ability to use good judgment to conduct professional and collaborative activities and solve practical problems within established limits and guidelines. Ability to recognize instances in which consultation with Supervisors, Director(s), or Senior Director should be sought before a decision is made.
Job Type: Part-time
Pay: $39.00 - $47.00 per hour
Expected hours: 20 – 40 per week
