JOB SUMMARY
Provide qualified support and care to patient/family in home health services, focusing on the resource management and issues directly related to the purpose the patient is on home health, and issues effecting the goals of treatment.
RESPONSIBILITIES
- When medical social services are provided, they shall be provided by a certified Social Worker (CSW) or by a social service worker (SSW) supervised by a certified Social Worker, in accordance with the Plan of Care. All social workers (LCSW, CSW, or BSW) report directly to the Clinical Manager.
- The Social Worker shall be responsible to:
- Assist team members in understanding significant social and emotional factors related to healthproblems
- Participate in the development of the Plan of Care, if needed
- Prepare clinical notes according to rules and agency policy
- Utilize community resources
- Participate in in-service programs
- Assist in explaining the Home Health program to the patient/family when necessary
- Participate, as appropriate, in the orientation, training, and support of Home Health Volunteers
- Attend in-patient discharge rounds, when appropriate, to help arrange discharge planning forHome Health patients
- Assist in discharge from Home Health services at request of Home Health Clinical Manager
- Provide grief counseling, as needed, to families after a death
- Keep accurate, up-to-date records to be included in the medical record for each patient/family
- Provide education to Home Health staff, volunteers, and community on topics related to Home Health care and social work, as schedule allows
WORKING ENVIRONMENT
Works indoors in the Agency office and patient homes and travels to/from patient homes.

REQUIRED SKILLS
- Candidate must have strong oral and written communication; organizational and problem-solving skills.
- Candidate must be detail and goal-oriented individuals and possess the ability to work independently and also collaborate with various healthcare representatives and agencies.
- Candidate must communicate well with doctors, nurses and other healthcare personnel and agencies.
- Candidate must understand various medical conditions and terminology.
- Candidate must be able to evaluate the mental, emotional and physical status of patients.
- Candidate must be skilled in providing bereavement counseling services to families.
- Candidate must understand federal, state and local healthcare standards and regulations.
- Candidate must possess current state licensure.
- Candidate must understand basic technology including the use of tablets and computer applications.
