** Starting Pay Range: $60,000 to $70,000 yearly ** Position may qualify for student loan repayment program.

We're seeking a dedicated Mental Health Counselor to join our team in supporting the well-being of an American Indian population in an outpatient setting. In this role, you'll conduct mental health assessments and provide compassionate treatment, as well as perform substance use evaluations and facilitate referrals for advanced care when necessary. Work Experience

What you will do as a Mental Health Counselor:
  • Provide mental health counseling to American Indian population as well as some substance abuse therapy.
  • Conduct interviews and assessments of patients and develop corresponding treatment plans to address issues relating to both mental health and addictions.
  • Work with other agencies to coordinate joint treatment and outside consultation and evaluation.
  • Provide thorough documentation in case records including the initial assessment, treatment plan, progress notes, discharge summary, correspondence and other related information.
  • Maintain up-to-date knowledge of financial and community resources available for mental health and addictions.
  • Maintain standards of professional conduct.
  • Make referrals for treatment for addictions and mental health and corresponds with referral sources and other community agencies.
  • Coordinate discharge planning activities for patients regarding addictions treatment or mental health treatment.
  • Provide prevention education and activities to American Indian Youth and Adults.
  • Provide timely and succinct feedback to primary care providers regarding consultation findings and recommendations.
  • Triage and referral of patients to off-site mental health and addictions services as appropriate.
  • Conduct Behavioral health interventions appropriate to the primary care setting.
  • Work as a primary care team member to develop clinical pathways or best practice programs for targeted patient groups (patients with diabetes, chronic pain).
  • Share knowledge with other team members and patients, both formally (in-services, consult reports, clinical team meetings) and informally (hallway conversations).
  • Responsible for development, teaching and/or oversight of classes promoting education and skill-building to enhance psychosocial and physical health.
  • Provide brief follow-up, including relapse-prevention education.


To be successful as a Mental Health Counselor, you'll need:
  • Master's Degree in Social Work or related field.
  • LMSW, LSCW, LMFT, LMHC, or LCAT.
  • CASAC preferred
  • Two to five years' experience as a mental health counselor.
  • Two to five years' experience in substance abuse evaluation/treatment.
  • Ability to communicate with clients in a non-threatening manner and to work effectively as a member of an organizational team.
  • A valid New York State driver's license.
  • To be willing to travel for training.
  • Comprehensive knowledge of computer programs, including Microsoft Word.
  • Knowledge of duties and responsibilities as a mandated child abuse reporter.
  • Ability to stand/walk for long periods of time and lift up to 10 lbs.
