What You’ll Do:
- Conduct client assessments and create personalized care plans.
- Coordinate medical, home care, and community services.
- Advocate for clients with healthcare providers and families.
- Provide crisis intervention and family caregiver support.
- Maintain HIPAA-compliant records using EHR software.
- Bachelor’s or Master’s in nursing, social work, gerontology, or related field.
- 2+ years in care management or geriatric care.
- ALCA membership or certifications (CMC, CCM) preferred (or willingness to obtain).
- Strong communication, organization, and problem-solving skills.
- Valid Illinois driver’s license and ability to pass background checks.
