1776 Senior Care is seeking a compassionate Care Manager to provide expert care coordination and advocacy for seniors and individuals with disabilities in Glen Ellyn and DuPage County. Join our team to make a meaningful impact in our community!
What You’ll Do:

Conduct client assessments and create personalized care plans.
Coordinate medical, home care, and community services.
Advocate for clients with healthcare providers and families.
Provide crisis intervention and family caregiver support.
Maintain HIPAA-compliant records using EHR software.

What We’re Looking For:

Bachelor’s or Master’s in nursing, social work, gerontology, or related field.
2+ years in care management or geriatric care.
ALCA membership or certifications (CMC, CCM) preferred (or willingness to obtain).
Strong communication, organization, and problem-solving skills.
Valid Illinois driver’s license and ability to pass background checks.
