- Master's degree in social work preferred.
- Licensed as Social Worker in Virginia preferred.
- 0-2 years social work experience preferred.
- Works with other medical professionals to identify the psycho-social needs of residents and secure services that meet these needs.
- Maintains open communications with team members, residents, families, volunteers, community organizations, and other facilities.
- Conducts the Social Services assessment for residents as assigned.
- Coordinate admissions/discharges and hospital stays into appropriate level of service at the community.
- Assists residents in understanding their rights including Advance Directives; provides information, support, and forms to clarify and document wishes.
- Attends Resident Care Plan or Service Plan meetings representing the psycho-social needs of residents.
- Works with resident and family during a change in level of care, coordinating the move with the Household/Neighborhood Leadership in the new level of care.
- Keeps documentation for all referrals.
- Investigates and reports alleged or actual resident abuse, neglect, or misappropriation of property to appropriate agencies and team members.
- All other duties as assigned.
- Knowledge of sound, current, social worker procedures.
- Excellent computer skills with proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.
- Strong attention to detail and high regard for confidentiality.
- Ability to communicate well with residents and family members as well as community team members.
- Exceptional customer service skills.
- Frequently exercises authority. Work is non-standardized and varied and requires incumbent to produce expected results and meet deadlines by determining criticality of the situation and which procedures to follow.
- Extensive walking, bending, stooping, standing.
- Ability to work in a fast-paced environment with multiple tasks.
- Ability to work inside and outside to effectively make rounds daily throughout the facility and to respond to problems and emergencies.
- Universal staffing, holiday, inclement weather, and a flexible work schedule are required.
