Our services encompass 14 supportive housing sites, partnerships with rapid and long-term rehousing providers, and first-class behavioral health services.

This is a new program! An exciting permanent housing intervention focusing on rehousing SIP guests. Participants are provided permanent subsidies for private market units and offered housing stabilization services to support the integration into new housing. Under the direction of the Housing Coordination and Retention Manager, the Behavioral Health Clinician provides ongoing intensive stabilization support to seniors (60+) referred to the FHSP program. The Behavioral Health Clinician II works in partnership with the Housing Coordinator and Retention Specialist to ensure the participant integrates into housing successfully.

Responsibilities:

In this role, you will:

  • Maintain a regular caseload and deliver professional, culturally relevant support, Work closely with the Associate Director of Behavioral Health, and the Administrative Assistant/Quality Assurance Specialist to reach all Medi-Cal billing goals and outcomes.
  • Maintain a regular caseload and provide professional and culturally relevant support, advocacy, information and referrals to program participants in order to guide them through a process of self-directed goal-setting that builds upon client strengths and talents.
  • Complete initial and ongoing assessment, diagnosis and treatment planning in collaboration with assigned caseload.
  • Meet with each client regularly and provide Medi-Cal billable mental health services including assessment, diagnosis, referral, consultation, brokerage, counseling and crisis management. Develop, review and revise treatment plans of care with clients based on individual needs. Perform all paperwork punctually, in accordance with Medi-Cal and CBHS timelines.

Requirements:

  • A master's degree in social work or a related field is required, along with Board of Behavioral Sciences registration/licensure (AMFT, APCC, ACSW, LMFT, LPCC, or LCSW).
  • 1+ years of providing therapy
  • A strong commitment to and knowledge of harm reduction practices.
  • Excellent written, verbal, and interpersonal communication skills.
