JOB CATEGORY: CARE COORDINATION / CASE MANAGMENT / SOCIAL WORK

POSITION SUMMARY: Under supervision, this position assists the Integrated Care Team (Primary Care Physician, Nurse/Medical Assistant, Behavioral Health provider, Care Coordinator II, Community Health Worker, etc.) by proactively coordinating care for patients to facilitate the achievement of quality and cost-effective health outcomes.

Qualifications

REQUIREMENTS

EDUCATION AND/OR EXPERIENCE:
  • Bachelor's degree in social work, nursing or other healthcare profession; OR
  • High school diploma or GED and minimum two years' healthcare experience.
  • Self-starter with strong verbal and written communication skills.
  • Comfort with change and quality improvement processes.
  • Experience with Electronic Health Records strongly preferred.
  • Experience with Motivational Interviewing strongly preferred.
  • Knowledge of New Mexico and local community resources strongly preferred.
  • Bilingual (English / Spanish) or (English / Navajo) preferred.

SCREENING REQUIREMENTS:

This position requires successful completion of an initial post-offer of employment:
  • According to PMS Human Resources Policy 205.006, "Healthcare Personnel Testing & Immunization Requirements," this position must comply with required testing and immunizations. Testing and immunizations including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella, will be available at the employee's PMS health center. Influenza vaccines will be required according to dates set by the CDC and NMDOH.
  • Criminal Records Check.
