About Job

Habitat Health is seeking an organized and compassionate Intake Coordinator to guide prospective participants through the PACE enrollment journey. This on-site role focuses on coordinating home visits, supporting additional assessments and leading center tours to help enroll new participants. Intake Coordinators will also help ensure a smooth transition to the Interdisciplinary Team (IDT). The Intake Coordinator plays a key role in facilitating the enrollment process for prospective participants, as well as connecting outreach and clinical teams while delivering an excellent experience for older adults and their families. Core responsibilities include working with cross-functional teams to enroll prospective participants, scheduling and coordinating home visits and follow-up appointments, communicating daily status to outreach & enrollment teams, collaborating with clinical teams for additional assessments, serving as a guide during center tours, tracking and managing leads through CRM and EMR systems, managing participant records and enrollment requirements, ensuring timely participant handoffs to IDT, supporting on-site events and enrollment conferences, and maintaining compliance with all policies. Required qualifications include experience in healthcare or enrollment sales, experience with State/County eligibility workers, proficiency in CRM software preferred, experience working with frail elderly, ability to work with minimum supervision, B.S./B.A. degree preferred, ability to lift up to 25 pounds, CPR/BLS certification or ability to obtain within 30 days, valid CA driver’s license and personal transportation, medical clearance for communicable diseases, and current immunizations. Preferred qualifications include bilingual skills in Spanish, Mandarin, or Cantonese. Compensation includes an hourly rate of $22 - $28, bonus eligibility, and benefits such as medical/dental/vision insurance, disability insurance, life insurance, flexible spending accounts, 401(k), paid time off, and company holidays. The role requires adherence to vaccination policies including COVID-19 and flu, with accommodations considered for disabilities or religious beliefs.
