Job Description

Social Worker Intake Coordinator

Location: Wayne, Oakland, and Macomb Counties | Full-Time | Travel Required

Position Summary

Under the supervision of the Director of Intake and Enrollment, the Social Worker Intake Coordinator is responsible for managing the full intake process for a comprehensive community-based healthcare program serving older adults. This includes conducting home visits and assessments, coordinating documentation for Medicaid eligibility, and facilitating communication between internal departments to ensure a smooth enrollment process for potential participants.

Key Responsibilities

  • Conduct in-home visits to meet with prospective participants and perform Social Work assessments to determine eligibility
  • Explain program benefits, complete intake assessments, and collect enrollment documentation
  • Submit written summaries and updates to the Intake Team based on home visit findings
  • Enter and maintain accurate participant information and assessments in the electronic medical records system (TruChart)
  • Coordinate closely with the Participant Advocate to prepare cases for the Interdisciplinary Team (IDT) review
  • Collaborate with eligibility specialists to ensure timely processing of Medicaid and Medicare documentation
  • Maintain ongoing communication with prospective participants and families through phone and written follow-up
  • Protect all confidential participant information and comply with organizational privacy practices
  • Perform other intake-related duties as assigned

Required Qualifications

  • Master’s Degree in Social Work (MSW)
  • Licensed in the State of Michigan (Limited or Full Social Work License required)
  • Minimum of 1 year of experience working with the elderly in a community-based setting
  • Knowledge of local senior services and community-based resources
  • Basic proficiency in Microsoft Windows, Outlook, and electronic recordkeeping systems
  • Excellent written, verbal, and interpersonal communication skills
  • Demonstrated ability to work both independently and as part of a team
