QUALIFICATIONS:

  • High School Diploma or equivalent
  • Social Service Designee Certification
  • Degree in Social Work, Psychology, Sociology, Gerontology is preferred
  • Excellent management skills
  • Good administrative and organizational abilities
  • Good interpersonal skills
  • Adheres to professional appearance
  • Knowledge of Social Resources in the Community

CLINICAL / ADMINISTRATIVE

  • Ensure the residents’ psychosocial and concrete needs are identified and met in accordance with federal, state and company requirements
  • Assess the psychosocial, mental and emotional needs of residents
  • Timely and accurate completion of the MDS
  • Orient residents to the center
  • Assist with discharge planning
  • Implement and update Resident Care Plan and Social History
  • Develop and maintain a list of community/outside resources
  • Communicate needs and plan of care to resident, families, responsible parties and appropriate staff
  • Maintain interpreter services for residents, if applicable
  • Assist with the facilitation of Advance Directives for those who indicate a desire to complete one
  • Maintain a Theft/Loss binder
  • Assist with the Grievance protocol
  • Participate in abuse investigations, when required
  • Participate in meetings, in-services and training as required
  • Implement and maintain an ongoing Quality Assurance Performance Improvement (QAPI) as required
  • Assist with coordination of resident room moves
  • Participate in the facility’s Behavior Management program
  • Provide Trauma Informed Care Screenings and Training per regulatory guidelines
  • Arrange ancillary services that have been determined necessary to maintain the residents’ concrete needs
  • Coordination and maintenance of the IDT invitation process to residents and resident representatives
  • Meet with consultants as required

CONSUMER SERVICE

  • Presents professional image to consumers through dress, behavior and communication skills
  • Adheres to company standards for resolving consumer concerns
  • Promotes and observes resident rights per regulatory guidelines
  • Maintains an organized office
  • Organize and maintain a professional work setting
