Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and resources to individuals and families facing various challenges. This role involves working in diverse settings to deliver essential services that promote mental health and well-being.
Duties
- Conduct assessments to identify clients' needs and develop appropriate care plans.
- Provide psychotherapy and counseling services to individuals, families, and groups.
- Facilitate group therapy sessions aimed at addressing specific issues such as PTSD care and crisis intervention.
- Engage in case management by coordinating services and resources for clients.
- Collect and analyze data related to client progress and program effectiveness.
- Collaborate with healthcare professionals, educators, and community organizations to ensure comprehensive care.
- Offer home community care support to clients in their living environments.
- Maintain accurate documentation of client interactions and treatment plans.
Experience
- A degree in Social Work or a related field is required; LCSW licensure required
- Experience delivering psychological services to children, adolescents, and young adults with intellectual disabilities preferred.
- Experience in special education or working with diverse populations is a plus.
- Proficiency in crisis intervention techniques and case management practices is essential.
- Familiarity with data collection methods for evaluating client outcomes is beneficial.
- Previous experience in hospital settings or providing group therapy will be advantageous.
