Overview
The Veterans Directed Care Coordinator will assist the Veterans to live as independently as possible by providing person centered care coordination to the Veterans in the VDC Program. The coordinator will assess daily living needs to determine amount of weekly care hours needed. Assist in devlopment of budget and spending plan for self-directed services and update as needed, monitor services that are being provided and budget is being followed, and help connect veterans to there social service resources as necessary.
The coordinator will provide information and assistance to Veterans and their representatives, meet with each Veteran referred to VDC to learn about the Veteran's needs (conduct an assessment to determine Functional needs) and learn how the Veteran would like to meet those needs. Prepare emergency backup plan for Veteran. Submit Veteran spending plans and amendments for approval. Report when a critical incident is identified with Veteran. Receive approved spending plans for distribution to the Veteran or the representative and monitor the veteran's monthly spending using report. Monitor Veterans overall health and safety in the home using monthly calls and quarterly home visits. Contact community resources on behalf of Veteran and make appropriate referals. Perform monthly Check ins with quarterly in person home visits to determine quality and quanity of services provided and veterans health status and if there are additional needs for the veteran.

Pay: $20.00 per hour
