Position Summary
Develops referral sources, obtains and reviews referrals, and coordinates admissions to skilled nursing facility.
Responsibilities
Develops collaborative relationships with hospitals, physicians, community organizations, team members and other referralsources.
Develops and implements a marketing plan that will lead to referrals and admissions.
Analyzes referrals to determine if the patient meets admission criteria both medically and financially.
Coordinates the patient’s discharge from the hospital, home, or other setting and facilitates admission to skilled nursing facility.
Prioritizes potential admissions based on the patient’s medical needs, financial support and discharge potential.
Meets with residents/families to explain the admission’s agreement and obtain signatures.
Coordinates opportunities for community groups to meet at skilled nursing facility.
Represents skilled nursing facility at community events.
Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.
Maintains compliance with all Orlando Health policies and procedures.
Other Related Functions
Enters referral information into the electronic referral log.
Verifies insurance coverage and obtains authorization.
Drives personal car and provides a monthly mileage expense report for reimbursement.
Qualifications
Education/Training
Associate degree, Nursing Degree from an accredited college or university, or graduate of an approved LPN program
Licensure/Certification
None
Experience
One (1) year experience marketing in the health care field.
