Education: Master’s Level
License: Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselors (LMHC), Licensed professional counselor (LPC), Licensed Practical Nurse (LPN), Registered Nurse (RN), or similar preferred.
About Us & Job Summary: Connections Care Management is a boutique care management firm that helps older adults, individuals with disabilities and families overcome obstacles related to acute or chronic health conditions. We are seeking a dynamic, compassionate and skilled professional to join our team and help our clients attain the best possible quality of life . We do this through home/facility/hospital visits, medical care coordination, advocacy, supportive counseling and innovative problem-solving. The ideal candidate can work independently while being a team player, has deep interest and experience in serving vulnerable adults and their families, an has existing knowledge of community resources, The job requires home visits primarily in the Essex, Morris and Bergen County areas.
Key Responsibilities:
- Conduct home, facility, and hospital visits to assess client needs and provide ongoing care coordination.
- Develop and implement care plans in collaboration with clients, families, and other healthcare providers.
- Advocate for clients by ensuring access to necessary medical, social, and community resources.
- Offer supportive counseling and assist clients and their families in navigating difficult decisions.
- Stay informed on local community resources, services, and programs to provide up-to-date guidance to clients.
- Monitor and enhance a client’s well-being with regular visits, counseling and support
- Accompany clients to medical appointments to ensure good communication, coordination and follow through of recommendations
- Advocate on each client's behalf with doctors, hospitals and health care facilities
- Maintain ongoing and immediate communication with family members.
- Offer counseling to help families reach agreement on the care of loved ones
- Screen, arrange and oversee in-home services for those clients who need them
- Make recommendations for senior housing, assisted living, nursing homes and specialized care communities
- Assist in managing mail, bills and other paperwork.
- Assist with applications for state and private benefits, and evaluate costs for those services
- Provide emergency crisis management and intervention as needed
