Position Overview: As a Back@Home Housing Case Manager, you will play a vital role in helping individuals and families secure stable housing while providing the support they need to never return to homelessness. Your expertise and compassion will make a meaningful impact as you work with our clients to overcome housing barriers. With your guidance, our clients will learn how to be good tenants, achieve mental health and housing stability, and foster key independent living skills to ensure a brighter future. The ideal candidate is detail oriented, a self-starter, and happy to work in a team atmosphere. Persons with lived experience, BIPOC, and bi-lingual are encouraged to apply.

Key Responsibilities:

- Conduct thorough assessments to understand clients’ needs and challenges.

- Develop and implement individualized housing plans with a focus on sustainable solutions.

- Provide supportive counseling and advocacy to empower clients in overcoming obstacles.

- Collaborate with landlords, housing agencies, and community resources to secure housing options.

- Maintain accurate and detailed case notes, ensuring the highest level of professionalism and confidentiality.

- Actively participate in team meetings, training sessions, and ongoing professional development opportunities.

Qualifications:

- Combination of Bachelor’s degree in Human Services or experience working in human services or lived experience

- Knowledge of local housing resources, tenant rights, and fair housing practices

- Excellent interpersonal and communication skills

- Ability to work independently and collaboratively within a team

- Cultural competence and sensitivity to the needs of diverse populations

- Must have a valid driver’s license with a 5-year clear driving record

- Must be able to lift 50 pounds
