About Job

Bright Future PPEC is a specialized pediatric extended care center (PPEC) committed to supporting children with complex medical needs and their families through comprehensive, compassionate, and coordinated care. We provide skilled nursing and therapeutic services in a nurturing day-health setting, creating brighter futures for medically fragile children and peace of mind for their families.

We are currently seeking a Licensed Clinical Social Worker (LCSW) to join our care team on a part-time and remote basis. In addition to providing vital psychosocial support, counseling, and advocacy for medically complex children and their families, this role will play a key part in helping expand access to our services. The LCSW will actively refer eligible patients and families to Bright Future PPEC, helping raise awareness of the specialized care we offer and ensuring more families can benefit from our nurturing, comprehensive pediatric support. This outreach and referral component is essential to fulfilling our mission and growing our impact in the community.

Key Responsibilities:

· Identify and refer eligible patients and families to Bright Future PPEC services, helping expand access and increase awareness of the center’s offerings

· Provide weekly individual and/or family therapy sessions using developmentally appropriate, evidence-based modalities

· Conduct psychosocial assessments to understand family dynamics, mental health status, and support needs

· Develop and maintain individualized treatment plans in coordination with caregivers and the interdisciplinary care team

· Participate virtually in care planning meetings with clinical staff and caregivers

· Collaborate with healthcare providers, case managers, and external resources to ensure continuity of care

· Offer emotional support and counseling for families coping with chronic illness, trauma, grief, or adjustment challenges

· Maintain timely and accurate clinical documentation, including progress notes, treatment plans, and discharge summaries

Qualifications:

Education & Licensure

* Master’s or bachelor’s degree in social work, Psychology, or a related behavioral health field

*Active Florida licensure as a Licensed Clinical Social Worker (LCSW)

Experience

*Minimum 2 years of experience working with children, particularly those with complex medical, developmental, or emotional needs

*Preference will be given to candidates with current or recent experience in a hospital or healthcare system

*Experience supporting families in healthcare or pediatric settings strongly preferred

*Familiarity with electronic health records and telehealth platforms

Skills & Attributes

*Strong clinical assessment and therapeutic skills

*Compassionate and culturally sensitive communication

*Ability to guide families through psychosocial challenges and care coordination

*Proficiency in Microsoft Office and standard EHR systems (e.g., TenEleven or similar)
