OUR WORKPLACE:

We are a small but dynamic Hospice agency focused on Quality and Growth. Our company is committed to providing the highest quality of care and services assuring human dignity and quality of life for our patients, their families, and our employees.

Primary Responsibilities will include:

-Assisting the physician and other members of the patient care team to develop an appropriate plan related to the terminally ill patient’s care

-Providing psycho-social support to the patients and families based on an assessment that identifies the needs, goals, interventions and services in accordance with the established plan of care.

-Assess and reassess social and emotional factors in order to help the patient and family to cope with problems of end-of-life care and to help them follow the hospice team’s recommendations to maximize the patient’s quality of life.

-Counsel patients and families concerning financial concerns, life review, positive family relationships and quality of life, care giving issues

-Recognize, identify, and utilize community and family resources to assist in maintaining independence and quality of life

-Attend and actively participate in patient care conferences to ensure good coordination of care

-Develop, prepare and maintain individualized patient care progress records with accuracy, timeliness and according to policies.

-Coordinate bereavement programs for families as well as community programs as needed.

-Participates in discharge planning, as appropriate

Qualifications

-Master’s Degree in Social Work

-One year experience as a Social Worker in a healthcare and/or hospice setting is preferred
