Job Description:

We are seeking a compassionate and dedicated Social Worker to join our team at Optalis of Troy. The Social Worker will be responsible for providing support and assistance to individuals and families in need. This includes conducting assessments, developing care plans, and connecting clients with resources and services to improve their overall well-being.

Responsibilities:

• Conduct assessments to identify clients' needs and develop care plans
• Provide counseling and support to individuals and families
• Connect clients with community resources and services
• Advocate for clients' rights and best interests
• Collaborate with other healthcare professionals to ensure comprehensive care

Qualifications:

• Bachelor's or Master's degree in Social Work
• Licensed Social Worker in the state of Michigan
• Previous experience in a social work or related field
• Strong communication and interpersonal skills
• Ability to work effectively with individuals from diverse backgrounds
