AgeWell Mental Health WI is dedicated exclusively to supporting the mental and emotional well-being of seniors in nursing homes, assisted living, and private homes throughout the Madison area. We give you the freedom to practice on your terms so you can focus completely on meaningful care.

What We Offer:

• Consistent Referrals – A steady flow of senior clients in nearby facilities and homes, often grouped efficiently to reduce travel.
• Flexible Schedule – Decide your own hours and client load.
• Zero Admin Burden – We handle all billing, insurance claims, credentialing, and Medicare enrollment.
• Onboarding & Training – Comprehensive guidance on EMR, documentation, and age-focused therapy.
• Deep Purpose – Help seniors deal with loneliness, grief, depression, cognitive changes, chronic conditions, and life transitions.

Your Role:

• Provide therapy for older adults across settings—nursing homes, assisted living, and private residences.
• Use evidence-based methods (CBT, Motivational Interviewing, etc.) tailored for the aging population.
• Develop individualized treatment plans and maintain HIPAA-compliant documentation in our EMR.
• Collaborate with family members, healthcare teams, and facility staff.
• Address needs such as chronic pain, adjustment to aging, grief, anxiety, and social isolation.

Requirements:

• Master’s in Social Work, Counseling, Psychology, or related field.
• Valid Wisconsin license: LCSW, LPC, or Licensed Psychologist.
• Experience with older adults strongly preferred.
• Skilled in communication, EMR documentation, and case management.
• Deep empathy, cultural competence, and dedication to serving seniors.
