Job Summary

The Family Care Coordinator will provide trauma-informed and culturally responsive assessment, ongoing service provision, and care coordination for caregivers and families referred to EMBARK. Ongoing services include the provision of prenatal and parenting education, concrete services, resource linkages, and care coordination with multidisciplinary team members to aid in the development of positive parenting skills and improved health and safety of the referred child and family. The Family Care Coordinator will meet with families in the home and community settings for face-to-face sessions but may also provide services and support via telephone and telehealth. Services will be tailored to address the risks and unique needs associated with maltreatment which may include increasing positive parenting strategies, relapse prevention, vocational/educational skills, budgeting, housing safety and habitability, and referrals to recommended treatment services and supports. Family Care Coordinators will provide direct services in accordance with agency-approved evidence-based practice models and program guidelines. The Family Care Coordinator must have a bachelor's degree in Social Work (or other related fields) with the ability to provide in-home services tochildren and families. Experience working with complex and multi-need families is a plus.

Responsibilities of Case Manager / Family Care Coordinator

-Establish initial contact with families within 24 hours of referral to assess safety,

-Initiate safety planning, and connect families with resources to meet immediate needs;

-Conduct team meetings and maintaining communication with the family and providers

-Facilitating the implementation of each family’s Action Plan

-Make frequent and consistent contact with DCBS personnel

-Provide direct services and education targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines.
