- Responsible for coordinating patient intake for post-acute services, including behavioral health, rehab and swing bed
- Conducts clinical assessments, facilitates patient registration, and ensures accurate documentation in alignment with organizational policies and standards
- Conduct clinical assessments for self-admit and referral patients to determine service needs
- Coordinate patient intake for services, including behavioral health, rehab and swing bed
- Collaborate with interdisciplinary team members to ensure seamless patient transitions
- Maintain accurate and thorough documentation of patient information, including medical history and consent forms
- Verify patient insurance information and ensure proper authorization for services
- Participate in assisting community outreach and marketing efforts as needed
- Excellent verbal and written communication skills
- Strong interpersonal and organizational skills
- Ability to work effectively in high-stress, fast-paced environments
- Proficiency in Microsoft Office Suite and Electronic Medical Records (EMR) systems
- Critical thinking skills and ability to make sound judgments under pressure
- Frequently required to sit, stand, and walk
- Occasionally required to lift or move items up to 25 lbs
- Exposure to blood, bodily fluids, and other potential biohazards
- Clinical setting with exposure to patients and healthcare providers
- Potential exposure to infectious diseases and hazardous materials
- Requires adherence to safety and infection control protocols
- High School Diploma or GED required
- Bachelor’s degree in a related field preferred

- Minimum of one year of experience, preferably in a clinical or case management setting
- Knowledge of healthcare registration, intake, and discharge processes
