Position Summary:

The Hospice Spiritual Counselor is responsible for assessing and addressing the spiritual needs of patients and families. This role is part of the interdisciplinary hospice team and collaborates with nurses, social workers, and bereavement counselors to ensure holistic care.

Key Responsibilities:

  • Provide spiritual support, guidance, and counseling to hospice patients and their families.
  • Conduct initial and ongoing spiritual assessments.
  • Support patients of all faiths, cultures, and belief systems, including those without religious affiliation.
  • Coordinate visits with community clergy as requested.
  • Participate in interdisciplinary team meetings and collaborate in care planning.
  • Maintain timely and accurate documentation in compliance with Medicare and agency standards.
  • Provide or assist with memorial services or end-of-life rituals as requested.
  • Offer bereavement support when needed.

Qualifications:

  • Master’s degree in Divinity, Theology, Pastoral Counseling, or related field.
  • At least one (1) year of experience in hospice, palliative care, chaplaincy, or related setting.
  • Strong communication, listening, and interpersonal skills.
  • Valid driver’s license, reliable transportation, and willingness to travel within service area.
