Position Summary

Essence Health Services, a trusted provider of home and community-based support services, is seeking a motivated and personable Community Liaison to represent the company across local events and gatherings. The Community Liaison will promote awareness of our services, build relationships with community stakeholders, and help connect individuals to care and support.

This is a per diem position requiring attendance at a minimum of two events per month across Bucks, Chester, Delaware, Montgomery, and Philadelphia counties, PA.

Key Responsibilities

• Represent Essence Health Services at designated events, including those hosted by community centers, rehabilitation facilities, healthcare organizations, and disability support networks.
• Speak confidently and accurately about the company’s full range of services, including:
• Non-medical home support services (personal care, companionship, light housekeeping, etc.)
• Community services (Community Integration and Community Support)
• Employment services for individuals with disabilities (Supported Employment, WEXA, and Pre-ETS)
• Veteran assistance and support
• Engage with attendees, providers, and partners to promote the organization’s mission and services.
• Submit a post-event report within 48 hours of each event, including:
• Confirmation of attendance (e.g., photos, sign-in sheets, etc.)
• Any event materials gathered (flyers, brochures, etc.)
• A summary of key interactions, feedback, opportunities, and next steps
• Identify and refer potential clients or partnership leads.

Compensation and Bonus Structure

• Paid a flat per diem rate equivalent to 8 hours of work for each event (minimum 4-hour attendance required)
• Earn a $300 bonus for each new client or referral generated through an event, once the client begins services and remains active for 30 days

Position Requirements

• Attend at least two community events per month
• Submit a complete post-event report within 48 hours
• Excellent communication and interpersonal skills
• Professional demeanor and a passion for serving the community
• Reliable transportation to travel to events across multiple counties
• Flexible availability, including occasional evenings and weekends

Qualifications

• Experience in community outreach, public engagement, or healthcare/social services preferred
• Knowledge of home care or disability support services is a plus
• Comfortable with public speaking and one-on-one engagement
• Organized, self-motivated, and able to work independently

Work Environment

This is a per diem, field-based position involving travel to various community events in Essence Health Services’ service regions across southeastern Pennsylvania.
