EveryMind is hiring a Care Manager to join our Bridge to Wellness team. This role is responsible for direct case management services within a school and community-based programs designed to integrate a system of resources and services for youth and families and to support them in achieving their goals.

Qualifications
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Strong Personal Resilience and Mental Health:
Given the high-stress nature of the role, it is essential that candidates possess a high level of personal resilience and maintain stable mental health. This ensures that they are well-equipped to provide effective support and maintain the professional standards required for client care.
Education: Bachelor’s degree in social service field required.
Experience: 1 year successful experience in the social services field. Experience with case management service delivery preferred. Experience working with youth and adolescents strongly preferred.
Skills: Strong interpersonal and communication skills. Demonstrated organizational and community building skills. Ability to manage multiple priorities. Accurate record keeping and data entry proficiency. Bilingual Spanish required.
Abilities: Effectively interact and work collaboratively in a flexible work environment with a variety of constituents including clients and family members or caretakers, interns, volunteers, paid staff, CEO, board members, and community partners. Ability to express ideas with clarity, confidence and sensitivity. Must be able to maintain confidentiality involving clients in accordance with organizational and federal guidelines.
Physical and Emotional Demands: The physical demands described here represent those that an employee must meet to successfully perform the essential functions of this position. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. While performing this job's duties, the employee is frequently required to stand, walk, sit, use hands to finger dexterity, handle with hands and arms, climb stairs, balance, kneel, crouch, or talk and hear. The employee must be able to lift and/or move up to 35 pounds frequently. The job's specific vision abilities include close vision, distance vision, and the ability to adjust focus.
Due to the nature of the content that is handled in the program, anyone who has a history of trauma or suicidality could be at risk of being triggered or re-traumatized. This is especially true for people who have experienced these issues within the past 12 months and therefore we may not recommend this work if their experience is so recent.
Clearance Requirements: The employee must pass a criminal background check. Certain job functions may require employee to pass a child sexual abuse and other criminal activity as well as a credit check. All staff providing services to staff or clients, must be free of illegal drugs and abuse of alcohol, and must be fully able to always perform all required duties.
Working Conditions: The work environment is representative of that which an employee encounters while performing the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
The employee must be available to perform the duties of the job during most, but not limited to core business hours (between 9-5pm, M-F), some flexibility with schedule may be available (evening and/or weekend hours). Employees will work mostly on-site but may work some hours remotely with the approval of supervisor.
The employee is also required to travel to different locations. Therefore, the employee must have access to reliable transportation.

- Completes comprehensive needs and strength assessments with youth and families resulting in individualized service plans and goals and appropriate crisis intervention or support as needed.
- Consults and collaborates effectively with colleagues, school staff, and other County/community agencies to provide comprehensive services for clients. Offers information and referral support for community members, school staff, etc.
- Provides on-going social services to clients which may include securing needed resources, providing necessary referrals, and ensuring any court-ordered requirements are met. Assists with completing applications for programs and services that align with service plan goals.
- Maintains compliance with School and Community-Based Youth Services contract including:
- Meeting productivity requirements
- Maintaining minimum caseload requirements
- Timely submission of data through County electronic records system
- Conducts psycho-educational groups, educational workshops, and resource presentations as appropriate.
- Maintains accurate, thorough, and timely documentation of client records. Documents and evaluates services provided and client progress, updates service plan at regular intervals.
- Manages funds in assigned budget categories and utilizes assigned client needs funds to support service plan and client goal achievement.
- Assists with seasonal events, such as camp registration, school supply drive and holiday giving program.
- Provides translation and interpretation services for clients and others as time allows.
- Assists with orientation and training of new staff.
- Utilizes regular supervision with Manager to evaluate and improve the quality of case management services and ensure that services are culturally appropriate for the client.
- Maintains client confidentiality according to EveryMind Confidentiality Policy.
- Adheres to EveryMind and Youth and Family Services Division (YFS) policies established by EveryMind Board of Directors, Chief Executive Officer, Chief Program Officer, and Division Director.
- Consistently represents EveryMind and the SCYS program in a professional manner. Treats all students, families, community members, school personnel, partners, donors, teammates and others with respect.
- Other duties as assigned.
