Horsham Center for Jewish Life is searching for an experienced Social Worker to join our team!

Long term care experience with the geriatric population is required for this position.

Job Type: Full Time, Mon-Fri 8am-4:30pm

Social Worker - Job Responsibilities:

  • Orients resident and family to nursing center, helping both adjust to new environment, and offers support as needed
  • Completes social history and MDS assessments, and develops care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken
  • Conducts/participates in resident care conferences with resident, family, and appropriate staff
  • Advocate and promote resident rights
  • Receives complaints from residents and families, forwarding complaints to appropriate staff to ensure resolution. Documents complaints. Follows facility policies and procedures
  • Performs other related duties, as assigned

Education & Qualifications

  • Must possess at a minimum a bachelor's degree from accredited school in Social Work or related field.
  • Experience in long-term care is required
  • Must be knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care
  • Must possess the ability to deal tactfully with personnel, resident, family members, visitors, government agencies/personnel, and the general public.
  • Knowledge of elderly, ill and/or disabled.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Ability to work independently or part of a group.
  • Effectively communicate with others.
