Ginger Cove is currently seeking a Full-Time Memory Care Coordinator. The Memory Care Coordinator is responsible for working in accordance with established policies and procedures, and/or specific instruction from the Director of Social Services and Wellness. Responsible for planning, organizing, and overseeing all aspects of the Memory Care program to ensure high-quality, person-centered care for residents living with dementia and related cognitive impairments. This role includes leadership, staff training, family support, and the development of engaging, meaningful programming that promotes residents’ well-being and dignity.

Job Duties & Responsibilities

  • Develop and implement individualized service plans (ISPs) in collaboration with the clinical team and families.
  • Monitor residents’ needs and adjust care plans as appropriate.
  • Promote a supportive and safe environment that enhances residents' quality of life.
  • Lead and oversee the design of memory-enhancing activities and therapeutic programming.
  • Ensure that programming addresses cognitive, physical, emotional, and social needs.
  • Train and mentor care staff on dementia care best practices.
  • Provide ongoing coaching and education on person-centered care, behavior management, and communication strategies.
  • Coordinate staffing needs to ensure adequate coverage and continuity of care.
  • Foster a positive team culture that emphasizes empathy, respect, and collaboration.
  • Serve as primary point of contact for families of Memory Care residents.
  • Conduct family meetings and support groups as needed.
  • Communicate regularly with families regarding resident well-being and program updates both in person and using our communication platform.
  • Promote community outreach and education regarding memory care and dementia.
  • Ensure compliance with state and federal regulations and company policies.
  • Monitor quality metrics and take proactive steps to address areas for improvement.
  • Participate in audits, surveys, and inspections as required.
  • Maintain accurate records and documentation.

Job Qualifications

  • Bachelor’s degree in social work, nursing, gerontology, or related field preferred.
  • Certification in dementia care or willingness to obtain (e.g., CDP – Certified Dementia Practitioner).
  • Minimum 2–3 years of experience in memory care, assisted living, or skilled nursing.
  • Experience leading teams or supervising staff is highly preferred.
  • Strong understanding of dementia, Alzheimer’s disease, and person-centered care principles.
  • Excellent leadership and interpersonal skills.
  • Strong organizational and time management skills.
  • Ability to communicate effectively with residents, families, and staff.
