Ginger Cove is currently seeking a Full-Time Memory Care Coordinator. The Memory Care Coordinator is responsible for working in accordance with established policies and procedures, and/or specific instruction from the Director of Social Services and Wellness. Responsible for planning, organizing, and overseeing all aspects of the Memory Care program to ensure high-quality, person-centered care for residents living with dementia and related cognitive impairments. This role includes leadership, staff training, family support, and the development of engaging, meaningful programming that promotes residents’ well-being and dignity.

This role includes leadership, staff training, family support, and the development of engaging, meaningful programming that promotes residents’ well-being and dignity.
Job Duties & Responsibilities
- Develop and implement individualized service plans (ISPs) in collaboration with the clinical team and families.
- Monitor residents’ needs and adjust care plans as appropriate.
- Promote a supportive and safe environment that enhances residents' quality of life.
- Lead and oversee the design of memory-enhancing activities and therapeutic programming.
- Ensure that programming addresses cognitive, physical, emotional, and social needs.
- Train and mentor care staff on dementia care best practices.
- Provide ongoing coaching and education on person-centered care, behavior management, and communication strategies.
- Coordinate staffing needs to ensure adequate coverage and continuity of care.
- Foster a positive team culture that emphasizes empathy, respect, and collaboration.
- Serve as primary point of contact for families of Memory Care residents.
- Conduct family meetings and support groups as needed.
- Communicate regularly with families regarding resident well-being and program updates both in person and using our communication platform.
- Promote community outreach and education regarding memory care and dementia.
- Ensure compliance with state and federal regulations and company policies.
- Monitor quality metrics and take proactive steps to address areas for improvement.
- Participate in audits, surveys, and inspections as required.
- Maintain accurate records and documentation.
- Bachelor’s degree in social work, nursing, gerontology, or related field preferred.
- Certification in dementia care or willingness to obtain (e.g., CDP – Certified Dementia Practitioner).
- Minimum 2–3 years of experience in memory care, assisted living, or skilled nursing.
- Experience leading teams or supervising staff is highly preferred.
- Strong understanding of dementia, Alzheimer’s disease, and person-centered care principles.
- Excellent leadership and interpersonal skills.
- Strong organizational and time management skills.
- Ability to communicate effectively with residents, families, and staff.
