Job Summary
We are seeking a compassionate and organized intake coordinator to manage the onboarding process for new home health patients. The candidate will coordinate initial referrals, gather necessary patient information, and facilitate smooth communication between clients, families and healthcare providers

Responsibilities

• Respond to referral inquires and assess patient needs
• Collect and verify patient information, medical histories, and insurance details
• Ensures maximum third party reimbursement through insurance verification and authorization processes.
• Coordinate with community liaison and business developer to ensure seamless intake
• Maintain accurate and up-to-date patient records in compliance with HIPAA and company policies
• Provide excellent customer service and support to patients and their families during the intake process
• Collaborate with the clinical team to prepare for patient care planning
• Assist with Durable medical equipment ordering

Experience

• LVN license required
• Previous experience in a healthcare setting, preferably home health setting
• Strong organizational and communication skills
• Ability to handle sensitive information with confidentiality
• Proficiency in computer systems and electronic health records (EHR)
• Compassionate, patient-focused, and detail-oriented
