Job Description

Home Location: 3003 N Central Ave., Phoenix, AZ

As a High-Risk Care Coordinator, you will facilitate the delivery of care by the Provider. Care Coordinators are liaisons to the patient/family/caregiver working with the Provider's direction and orders to provide quality healthcare for the home bound or home limited patient. The high-risk team is dedicated to caring for complex, high-needs patients to improve their quality of life, enable them to live well and age healthfully at home and avoid hospitalizations. Each team member will play an essential, role within the team. Our high-risk program offers individualized, evidence-based, multidisciplinary, value-based care to our patients with the greatest needs in the care setting best suited to support their wellbeing including the PCP office, high risk clinics, patient homes, and other settings (e.g., dialysis centers).

How You Will Get Things Done

  • Complete daily follow-up sheets including ordering of labs & tests, referrals, ordering of DME equipment
  • Order any acute imaging/labs for Provider
  • Provide support to designated provider in all patient care areas
  • Management of required signature documents
  • Procession of all completed documents (including outgoing faxes)
  • Track outgoing referrals to home health agencies
  • Communicating & collaborating with other Care Coordinators to assure all work is completed
  • Keep workspace within HIPPA compliance
  • Communicate daily with Provider to encourage delivery of comprehensive care plan to each patient within the team and expedite workflow to facilitate Providers ability to see patients
  • Check EHR email regularly (approximately every 2 hours). Follow up on requests in a timely manner
  • Schedule follow-up patient appointments daily
  • Run INSPECTS for Providers on any new patients and patients that are currently on narcotics for the next day’s schedule
  • Answers phone calls/messages from or pertaining to Village At Home patients
  • Phone in prescription refills

  • Communicate unexpected schedule delays to patients in a timely and empathetic manner and provide assistance to ensure patient comfort and satisfaction during any delay.

  • Serve as a Village brand ambassador, building strong relationships with patients, families, caregivers, and community leaders to enhance our reputation and visibility within the community.

Experience To Drive Change

  • Certified Medical Assistant or eligible within 90 days of hire
  • 3+ years of experience in a healthcare setting or a bachelor's degree in a healthcare related field.
  • Current BLS certification for healthcare providers preferred; required within 90 days of employment.
  • High School Diploma/Equivalency required.
  • Comfort with technology including the Microsoft suite of products
  • Prior experience using electronic health record
  • Experience in primary care, family practice, internal medicine, urgent care or ER preferred.
