Join Trinity Health PACE as a Social Worker to deliver compassionate and comprehensive social services to PACE participants and their families. You will plan, coordinate, and implement services that support physical, emotional, and social well-being, with a special focus on serving frail or elderly populations.
What You Will Do:
- Conduct assessments and develop individualized care plans
- Provide counseling, support, and education to participants and caregivers
- Collaborate with the interdisciplinary team (IDT) on 24-hour care coordination
- Facilitate resource referrals, financial eligibility, and advance care planning
- Lead support groups, family meetings, and crisis interventions
- Advocate for participants with external agencies and ensure quality care transitions
- Master’s degree in Social Work (MSW)
- Two years of experience preferred, including one year with elderly/frail populations (or willingness to undergo training)
- Experience in multidisciplinary teams and community-based care is a plus
- Working knowledge of Medicare, Medicaid, and social service principles
- Licensure in Social Work preferred
