- Hours: Nights 40hrs per week 11:30pm-7:30am Monday-Friday or alterative schedule with same hours but Sunday-Thursday.
- Conduct comprehensive biopsychosocial assessments.
- Develop and implement plans to address social determinants of health.
- Perform suicide risk, mental health, and crisis assessments and interventions.
- Document safety plans and care provided in patient medical records.
- Educate patients on and assist with completing advanced directives.
- Coordinate and complete the process for inpatient psychiatric petitions.
- Collaborate with interdisciplinary teams, communicating critical patient information.
- Plan and facilitate family and interdisciplinary team meetings, including goals-of-care discussions.
- Provide resources for homeless Veterans and address abuse or neglect findings in coordination with Adult Protective Services.
- Develop discharge plans, including referrals to rehabilitation, long-term care, or palliative care facilities.
- Assist with discharge planning for Veterans returning home, including home health consults.
- Manage case assignments for post-discharge case management services.
- Educate patients and families, ensuring they understand care instructions.
- Support emergency measures and provide life-saving interventions when necessary.
- Maintain professional decorum, respect patient rights, and uphold the VA's customer service standards.
- Review and utilize electronic medical records to inform service delivery.
- Education: Master of Science in Social Work (MSW) from a nationally accredited school.
- Experience: Minimum of two years of current social work experience.
- Licensure: Current, full, and unrestricted social work license in any U.S. State, Territory, or the District of Columbia.
- Certification: Basic Life Support (BLS) certified by an American Heart Association (AHA)-approved vendor
- Strong crisis intervention and mental health assessment skills.
- Expertise in discharge planning and connecting Veterans with appropriate resources.
- Proficiency in interdisciplinary collaboration and patient advocacy.
- Commitment to maintaining patient privacy and respecting individual rights.
- Competence with electronic medical records and documentation protocols.
