Case Manager (Social Worker)About The Company:

The position offers a pay rate of $35 per hour.Responsibilities:

Provide case management services for individuals in an addiction recovery facility.
Conduct assessments, develop care plans, and monitor patient progress.
Collaborate with healthcare providers, counselors, and community resources.
Support patients with social service needs, including housing and employment assistance.
Maintain accurate records and ensure compliance with relevant regulations.
Work flexible shifts to provide continuous support as needed.

Required Qualifications:

A Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.
1+ years working with individuals experiencing substance use, mental illness, addiction, or homelessness
Flexibility to cover various shifts, including nights and weekends.
Strong communication, organizational, and problem-solving skills.
Proficiency in case management software and electronic medical records is a plus.

Preferred Qualifications:

A Master’s in Social Work (MSW) or a Professional Social Work Certification (LICSW, LSW, LCSW).
