Location: Member-facing role, San Antonio, TX
Duration: 6 months
Work Hours: Monday–Friday, 8:00 AM – 5:00 PM
Worksite: Remote field-based
Position Overview:
The Flex Team Specialist will support the Star Kids program by providing comprehensive care coordination to pediatric members. This includes conducting in-home SAI assessments, telephonic outreach, arranging DME and therapies, and ensuring timely follow-ups. The role is designed to fill in for staff currently on leave.
Key Responsibilities:
- Conduct in-home SAI assessments
- Perform regular telephonic outreach and 28-day follow-ups
- Coordinate services such as DME, therapies, and provider referrals
- Document activities and complete required forms accurately and timely
- Meet all turnaround time (TAT) and documentation standards
- Follow established workflows and processes
Qualifications:
- Required:
- RN license
- Associate’s Degree
- Pediatric care experience
- Proficiency with Word, Excel, Outlook, and Microsoft Teams
- Strong communication and organizational skills
- Preferred:
- Bachelor’s Degree
- Prior experience with Medicaid LTSS populations and service coordination
Onboarding & Expectations:
- First day includes equipment setup and Centene training modules
- Performance measured by ability to complete outreach, assessments, documentation, and follow-up within set turnaround times
- Must be comfortable working independently and navigating multiple internal systems
