Key Responsibilities:

Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.
Participates in an interdisciplinary team approach to better serve the needs of the facility population.
Implementing and documenting a plan of care related to the resident’s social/emotional needs in the electronic medical record.
The Director of Social Services and Admissions is also responsible for collaborating with the nursing, MDS and rehabilitation departments to facilitate discharge plans for residents

Qualifications & Experience Requirements for Director of Social Work:

Previous Experience as a Director of Social Work preferred
Must have SNF or LTC experience
Valid Social Worker License
Must possess, as a minimum, a Master of Social Work from an approved school of Social Work.
Must have a minimum of 3 years of long-term care experience.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
Strong leadership skills in a healthcare setting
Strong written and verbal skills
