Case Management Job Description
Modani Care is an expanding home care provider located in Missouri. We are currently seeking a qualified candidate to join our Kansas City, MO, office. The role will involve assisting with case management and engaging with new patients to facilitate the completion of their documentation, as well as performing various administrative tasks to support our operations.
Responsibility’s
· Conduct daily evaluations to ensure that consumer services are delivered with accuracy and precision.
· Perform monthly follow-up calls to ascertain that the needs of consumers are being adequately addressed.
· Conduct an annual home visit to consumers to verify that their requirements are being met effectively.
· Address inquiries from consumers and caregivers, providing exemplary customer service.
· Maintain patient records with a high degree of accuracy and confidentiality.
· Welcome patients and visitors in a professional and courteous manner.
· Efficiently schedule patient appointments.
· Ensure compliance with HIPAA regulations and uphold patient confidentiality.
Requirements
· Familiarity with HIPAA regulations and best practices in patient services.
· Proficient communication skills and an aptitude for effectively engaging with patients.
· Prior experience and knowledge in the Home and Community-Based Services (HCBS) sector.
Job Type: Full-time
Pay: $21.48 - $23.54 per hour
Expected hours: 40 per week
