Position Summary

The Salvation Army Northern Division Harbor Light Center (HLC) and satellite locations is "Doing the Most Good" by providing shelter, housing, and holistic, person-centered, supportive services to over 300 single adults each night. HLC also offers worship services, pastoral care, and connections to community partners.

The Case Manager provides individualized case management to support adult participants in achieving housing, employment, and self-sufficiency goals. This role is responsible for maintaining accurate participant records, recruiting eligible participants, and promoting social services programs. Additionally, the Case Manager engages in community outreach, supports organizational initiatives, and facilitates educational opportunities to connect participants with valuable resources.

Essential Functions (approximate time spent)
  • Case Management (75%): Provide short- and long-term, strength-based, person-centered, and holistic case management for adult participants. Develop and implement individualized service plans, monitor progress, and support participants in achieving goals related to housing, employment, and self-sufficiency.
  • Participant Information Management (10%): Maintain accurate and up-to-date participant records in case files and databases. Ensure all case notes, assessments, and progress updates are documented in a timely and compliant manner.
  • Participant Recruitment & Program Promotion (5%): Recruit eligible participants for housing programs through outreach and referrals. Promote social services and corps programs to increase engagement and accessibility.
  • Community & Organizational Engagement (5%): Support and promote The Salvation Army and HLC initiatives by collaborating with colleagues and participating in community events, forums, and outreach activities.
  • Educational Opportunities & Resource Development (5%): Facilitate individual and group educational sessions. Contribute to the maintenance of a catalog of local community resources and service providers to support participant success.

$23.29-26.88/hour

Education and Experience
  • Bachelor's degree in a relevant field; a combination of education and experience may be considered in place of a degree.
  • State board licensure is required if the degree is in social work.
  • Previous experience in social services is required.
  • Experience with long-term case management is preferred.
  • Must successfully pass all required background checks.
  • A valid driver's license is preferred, and candidates must meet the MVR requirements.

  • Strong interpersonal and communication skills with the ability to engage effectively with diverse populations.
  • Demonstrates integrity, professionalism, and discretion when handling confidential information.
  • Highly organized with strong attention to detail.
  • Proficient in Microsoft Office, databases, and general PC use.


Working Conditions
  • Variety of work, sitting approximately 50% of the time with some prolonged standing, walking, and bending.
  • Must be able to lift to 20 pounds occasionally.
  • Work is conducted in a professional setting and is not eligible for hybrid, remote-friendly work arrangement.
  • Requires clear and effective communication with colleagues, participants, and community members.


Supervisory Responsibilities

None.

Driving/Travel Requirements

If approved, drive agency or personal vehicle locally to support the mission.
