Job Summary:
The Care Coordinator role assists families and staff with coordinating around their schedule and case starts. This role bridges the gap between client and employee scheduling/staffing and requires a strong commitment to delivering outstanding customer service to both families and staff. This role reports directly to the Regional Clinical Director.
Responsibilities:
- Reach out to families once their assessment is completed to confirm availability and discuss next steps in the process, including weekly check-ins, even with no new updates, to ensure that the family and staff know they are valued.
- Reach out to RBTs/BTs once they are hired to discuss caseloads and expectations.
- Accurate data entry and communication recorded in EHR and other software.
- Update client availability and employee availability in Central Reach.
- Schedule start dates for families, coordinate closely with clinical team, families and employees.
- Coordinate restaff dates with families as needed.
- Schedule and cancel sessions in Central Reach.
- Conduct monthly check-in with families and RBTs. Document conversations and do any follow ups needed.
- Issue corrective action as needed to RBTs for attendance, unconverted sessions notes, accurate data collection, etc. and document in Paylocity.
- Exercise discretion to schedule clients appropriately, maximizing their authorization.
- Audit all parts of EHR for accuracy and completeness.
- Evaluate staffing needs and communicate with team.
- Attend team meetings and communicate current needs.
- Identify scheduling and staffing adjustments necessary, and address accordingly based on client satisfaction feedback received.
- Engage in community outreach efforts, including building relationships with local daycares and other identified partners, to support continued growth.
- Other duties as assigned.
Qualifications:
- High School diploma or GED; higher education preferred (associate or bachelor’s degree).
- Detail-oriented and organized.
- Excellent verbal and written communication skills.
- Proficient with Microsoft Office Suite or related software.
- Previous experience with intake/care coordination.
