Project Specialist: Housing Case Manager provides ongoing support to program participants in an effort to support individuals to achieve and maintain housing stability.  This is a people-centered position. Key Duties include:

• Case management which includes developing individualized supportive service plans with clients and setting clear goals and timelines for achieving housing stability.
• Monitor client progress and provide ongoing support throughout the housing process.
• Coordinate with other service providers to address related needs like employment, mental health, or substance abuse treatment.
• Maintain internal support services which many include food pantry, clothing closet and other supports that contribute to housing stability for program participants.
• Perform acclimation visits and/or home checks with participants as may be required to ensure housing safety and stability.
• Conduct inspections using HQS or other appropriate standard to ensure housing quality and safety for program participants.
• Resource navigation which includes connecting clients with relevant community resources, such as utility assistance, furniture banks, and financial literacy programs.
• Training in SOAR, food stamp application process, Medicaid application process and other non-cash benefits that could promote housing stability with program participants.
• Document and maintain detailed client records, including progress notes, housing search activities, and communication with participants, community partners and/or landlords.
• Prepare reports on caseload activity and program outcomes. 

Qualifications

Combination of experience and education sufficient to successfully perform the essential duties of the job as described above. Degree or lived experience preferred but not required. Experience in case management or social service is strongly desirable.  Must have reliable transportation.
