1. Unit Name: Acute Care Case Management – RN Case Manager.

2. Brief Unit Description: Coverage for inpatient medical/surgical patients.

3. Years of Experience Required: 3+ years.

4. Skill Sets Required: Acute Care Case Management, Utilization Management, and Discharge Planning.

5. Clinician to Patient Ratio: 1:15.

6. Patient Population: Adult.

7. Bed Count: 200.

8. EMR (Charting System): EPIC.

9. Floating Required: Yes.

10. Rotating Shifts Required: No.

11. Certifications Required: Active Massachusetts RN License. BLS & ACLS. Certified Case Manager preferred.

12. Additional Information: Weekend commitment – every 6th weekend.
