SOCIAL SERVICES COORDINATOR

Objective of the position

Under the general supervision of the Executive Director, develop, coordinate, and administer services which will serve the social, educational, health, and recreational needs of the residents of the Housing Authority on an individual, family, and group basis. Promote and maintain working relationships with local social, educational, recreational, and health agencies, both public and private, to help Housing Authority residents access the supportive services they need to continue living as independently as possible in their homes. Interpret Housing Authority policy and regulations to residents, local agencies, and the public to encourage mutual understanding and cooperation for the benefit of all concerned.

HUD’s Service Coordinators program provides funding for service coordinators to assist elderly individuals and people with disabilities in obtaining the supportive services they need to continue to live as independently as possible in their homes. The service coordinator plays a critical role in supporting HUD-assisted housing as a platform for financial security, physical security, social connections, and the delivery of long-term community-based supportive services, especially for vulnerable populations such as the elderly and persons with disabilities

Functions of Work

  • Orientation and counseling of new residents on regulations, practices, responsibilities, facilities, and services available from the Housing Authority.
  • Provide information and referral services for residents with problems in such areas as Social Security payments, insurance, medical bills, health and housekeeping services, family problems, financial concerns, nutrition, transportation, etc.
  • Counsel with residents, (and their families), who are unable to meet conditions of occupancy such as rent payments, housekeeping, personal hygiene, reality orientation, physical and health impairments, etc., to determine the nature and extent of such problems, to assist the residents toward the resolution of issues such through personal action or referral to the proper service agencies, and to provide encouragement and support to the resident while such problems are resolved.
  • Coordinate community resources, including social services, private healthcare providers, visiting nurses, ADVOCAP, and Social Security, to provide maximum service to residents. Work with residents, social workers, and other agency staff to resolve resident problems.
  • Interview and screen prospective residents for placement in Housing Authority units. May include interviewing prospective residents at residencies, hospitals, or nursing homes before acceptance by the Authority. Assist with move-in and move-out inspections as directed. May consist of moving arrangements, nursing home placements, or eviction actions.
  • Assist with social programming for residents, including Christmas and other seasonal activities, tenant meetings, bake sales, bazaars, and other events, serving as a liaison between management, floor leaders, and resident organizations.
  • Work closely with the nurse of the Older Adult Health Care Program of the Oshkosh Seniors Center, particularly in areas of resident assessment, care planning, and general health/nutrition programming.
  • Fulfill the educational requirement as outlined by HUD.
  • Organize programs on topics of interest to residents. Distribute consumer materials often available free from organizations such as State and area agencies on aging, the American Association of Retired Persons, the National Council on the Aging, senior centers, Councils on Aging, legal services offices, or the services or programs themselves (such as Medicare and Medicaid).
  • Organize meetings to "teach" residents about housing development rules, regulations, and operations.
  • Help managers and residents identify and resolve safety or accessibility issues.

Position Qualifications

  • A Bachelor of Social Work or a degree in Psychology or Counseling is preferable. Supervisory experience may be necessary in some situations.
  • Two to three years of experience in social service delivery with the elderly and/or family populations.
  • Demonstrated working knowledge of supportive services and other resources in the area served by the project.
  • Demonstrated ability to advocate, organize, problem-solve, and provide results for the residents they serve
