About Job
- Coordinate patient care activities, including scheduling appointments, arranging referrals, and ensuring follow-up care
- Maintain accurate and up-to-date patient records in electronic medical record systems
- Communicate with patients, healthcare providers, and insurance companies to ensure seamless care coordination
- Provide individuals with physical, emotional, and behavioral support allowing them to actively participate in activities offered while encouraging personal choices and assisting in maintaining their personal safety
- Provide transportation, in personal and company-owned vehicles, throughout the day to and from the individuals' homes and activity locations in the community
- Educate patients on available resources and support services
- Customer service orientation-proactively assist clients in a professional, friendly manner to create the best possible experience by building relationships, understanding quality standards for service, and utilizing customer needs assessments
- Social perceptiveness-be aware of other people's reactions and understand why they react the way they do. Maintain a positive, friendly composure even in difficult situations
- Discretion-act with prudence and circumspection, especially in the handling of confidential and private documents/data.
- Self-motivated-work autonomously to set and achieve targeted goals
- $40,000-50,000/yr + Benefits
- Associate's Degree required; Bachelor's degree preferred in a field related to human services
- Previous experience in a patient support role is highly desirable
- Familiarity with medical terminology
- Bilingual Spanish-English
