DescriptionA Brief OverviewCollaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans.What You Will Do
  • Provides psychosocial assessment / intervention to assist patients/families.
  • Provides age specific care to patients/families in all age groups. * Develops and implements a comprehensive plan based on identified psychosocial needs and/ or discharge needs.
  • Develops plans with attention to individual patient and family goalsProvides linkage to community resources to facilitate optimal care * Communicates/collaborates with the work team using a multidisciplinary approach.

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
QualificationsEducation
  • Bachelor's Degree from an accredited school of Social Work (Required)
Work Experience
  • Hospital/Health Agency experience. (Preferred)
Knowledge, Skills, & Abilities
  • Computer skills (Required proficiency)
  • Theoretical and clinical knowledge regarding health care delivery (Required proficiency)
  • Multi-tasks and prioritizes work (Required proficiency)
  • Communicates effectively with persons of various backgrounds (Preferred proficiency)
Licenses and Certifications
  • Licensed Social Worker (LSW) (Required)
Position Details/Preferred QualificationsAt least one year of recent experience is strongly desired. This Social Worker will help substance abuse patients in the emergency department, by navigating various resources and providing continuity of care. Previous work with patients with opiod or other addiction is strong preferred.Physical Demands
