We are seeking a dedicated Hospital Social Worker (LCSW) to support patients and families in a high-acuity hospital setting. The ideal candidate will provide emotional, psychological, and resource-driven support during the evening shift.Key Responsibilities:
- Deliver crisis counseling and psychosocial evaluations
- Collaborate with healthcare teams for patient-centered care plans
- Assist in discharge planning and referrals to community services
- Advocate for patient welfare and regulatory compliance
- Document assessments and progress in the EMR system
- 2+ years of hospital-based LCSW experience
- Active GA LCSW license
- AHA BLS certification
- Acute care experience strongly preferred
