What We’re Looking For:

  • Compassionate and patient-centered approach to care coordination

  • Experience in hospital settings such as emergency departments, ICU, med/surg, and specialty units

  • Bachelor’s degree in Nursing, Social Work, or a related healthcare field required; Master’s preferred

  • Current licensure or certification in case management, nursing (RN), or social work (LMSW, LCSW) as required by role

  • Strong communication, assessment, and interdisciplinary collaboration skills

  • Familiarity with discharge planning, resource utilization, and federal compliance tools (e.g., HINNs, ABNs, MOONs)

  • Ability to coach, lead, and mentor interns and students where applicable

What You Will Do:

The Care Coordinator partners with medical staff and clinical teams to support patients through the full continuum of acute care. This role ensures each patient’s care plan is comprehensive, cost-effective, and centered on safe transitions across care settings.

  • Facilitate patient access to appropriate care and advocate for care aligned with patient needs and quality metrics

  • Conduct holistic assessments to develop progression-of-care management plans, identifying barriers and psychosocial risks

  • Implement patient-driven care management and collaborate with interdisciplinary teams to optimize outcomes

  • Serve as a liaison between patients, families, providers, payers, and external care teams

  • Monitor, document, and evaluate the care plan to ensure quality and financial efficiency

  • Lead discharge planning, referrals, and education to ensure smooth transitions and avoid readmissions

  • Provide supervision for interns and students in accordance with state practice acts

  • Participate in clinical huddles, patient care conferences, and contribute to continuous improvement initiatives

EXPERIENCE REQUIREMENTS

Prior experience in a hospital or case management role preferred.


EDUCATIONAL REQUIREMENTS

Current Louisiana RN licensure OR master’s in social work

LMSW OR LCSW required if a Social Worker.

SPECIAL SKILLS, LICENSE AND KNOWLEDGE REQUREMENTS

  • Ability to manage conflict, stress, and multiple competing work demands in an effective professional manner.
  • Knowledge and ability to manage multiple patients of cultural diversity and low literacy issues in care provisions.
  • Demonstrates knowledge of human behavior, socioeconomic factors in disease and illness, behavior patterns of the physically and mentally ill patient.
  • Demonstrates outstanding communication skills and can establish constructive relationships with patients, families, and hospital associates.
  • Demonstrates pro-active creativity in negotiating post-acute services with community providers and the finance department.
  • Works well under pressure of time and shifting priorities.
  • Demonstrates skill in written documentation and providing concise information to the medical team to facilitate care delivery.
  • ACM or CCM certification preferred.

HIPAA REQUIREMENTS:

Maintains knowledge of and adherence to all applicable HIPAA regulations appropriate to the job position, including but not limited to: Medical records without limitation of both paper and electronic, patient demographics, lab and radiology results, patient information related to surgery or appointment schedules, information related to patient location, religious beliefs and/or public health records, medical records related to quality/data, patient financial information and/or 3rd party billing, patient-related complaints, research information, employee health records and employee prescriptions.

SAFETY REQUIREMENTS:

Maintains knowledge of and adherence to all applicable safety practices appropriate to the job position, including but not limited to: Incident reporting, PPE, exposure control plans, hand washing, environment of care, patient identification.
