About Job

The Missouri Hospital Association is seeking a motivated self-starter to work closely with Missouri hospitals, health care systems and associated partners to design, develop and operationalize care coordination and referral service connectivity to address high priority issues affecting maternal-infant populations. Specifically, identifying new and supporting and/or strengthening existing referral and care coordination pathways for maternal patients with substance use disorders and other mental health diagnoses is a critical focus of this role. The selected candidate will assist the Vice President of Clinical Quality Improvement, and the Vice President of Substance Use Programming to manage the development and coordination of referral and care coordination networks with a goal of reducing severe maternal morbidity and mortality. This position is funded through a contract received from the Missouri Department of Health and Senior Services to reduce severe maternal morbidity and mortality. Continuation of this position is dependent on renewals of this or related contracts.

A minimum of an associate’s degree in nursing or a bachelor’s degree in public health, social work or other related field of study is required. Experience with navigating the clinical-community health continuum and an ability to build efficient networks is required. Experience in contract/grant management and implementation of defined scopes of work and evaluation of those scopes of work is preferred. Ideal candidate will have the ability to demonstrate a high comfort level in actively facilitating and participating in meetings and managing multiple complex projects simultaneously. Candidate will also regularly perform data processing duties including collection, editing, processing and community-based reporting of data. In addition, will provide education and training to clinical and community staff regarding evidence-based practices with a focus on CDC prescribing guidelines, screening, brief intervention, referral to treatment and referral/care coordination process. Must have experience and ability to demonstrate a high comfort level in actively facilitating and participating in meetings and managing multiple complex projects simultaneously. Must have the ability to analyze and explain quantitative data and demonstrate proficiency in Microsoft Office products. Ideal candidate must have strong communication and writing skills.

Job Type: Full-time

Pay: $57,680.00 - $65,000.00 per year
