Position Summary

The Program Nurse Care Coordinator is responsible for providing clinical support and facilitating patient participation in evaluation and treatment programs related to transplantation and dialysis or Cancer, ensuring ethical standards and optimal patient care. This position reports to Murse Care Management.

Principal Duties And Responsibilities

  • Secure, track, and review patient records, identifying relevant program-specific documents.
  • Utilize clinical judgment to prepare records for external reviews and triage patients for program candidacy.
  • Act as a liaison between Centers of Excellence (COE) facilities and Care Managers.
  • Educate members on program policies.
  • Collect and analyze data necessary for program enrollment and reporting.
  • Maintain positive relationships with external vendors and provide seamless customer service.

  • Adhere to established safety standards and utilizes proper techniques to avoid work-related injuries.

Knowledge, Skills And Ability Requirements

  • Associates or Bachelors degree in Nursing; current Arkansas nursing license required.
  • Strong organizational and communication skills.
  • Proficient in Microsoft Office applications.
  • Minimum of one year of clinical nursing experience, with three years preferred.
  • Experience in care management is a plus.
