Licensed Clinical Social Worker – up to $75/hourJob Summary:We are seeking a highly qualified Case Manager with LICSW or LCSW certification to lead treatment planning and discharge coordination for youths with developmental disabilities. Key Responsibilities:

Guide treatment teams in planning, care coordination, and transition processes.
Conduct psychosocial assessments and pre-admission interviews.
Evaluate outcomes and recommend changes to support individualized care goals.
Liaise with state-run departments and local districts to ensure alignment in services.
Provide patient and family education on adult services, insurance, and life skills.

Requirements:

3+ years of professional experience as a licensed social worker.
Active LICSW or LCSW certification.
Ability to work an on-site schedule in Canton, MA Monday-Friday 8am-430pm

Preferred Skills:

Experience in youth transitions and hospital discharge planning.
Social work experience with a youth population
