Location: Office in Holyoke, MA, serving members in Western Massachusetts (primarily Berkshire County)

As a Care Coordinator, you will play a key role on our interdisciplinary team—working to ensure that children and adults facing medical, behavioral health, or substance use challenges receive the support and services they need to thrive.
Schedule Requirements
- Monday through Friday position
- Staff are required to be in-office during the first six months of employment
- Standard shift options:
- 8:00 AM – 4:00 PM with a 1-hour lunch
- 9:00 AM – 5:00 PM with a 1-hour lunch
- New staff begin on the 9:00 AM – 5:00 PM schedule for training purposes
- Full-time = 35 paid hours per week
- After six months, the role becomes eligible for up to 4 remote workdays per week, with no less than 1 required in-office day
- Field visits to members are required weekly, regardless of work location
What You’ll Do
- Provide care coordination by phone and in person using evidence-based practices
- Support care transitions, including discharge planning
- Educate members about the Community Partner (CP) program and promote engagement
- Monitor and revise care plans in collaboration with providers and members
- Provide wellness coaching and connect individuals to community resources
- Develop and maintain relationships with community-based organizations and providers
- Serve individuals with empathy, cultural responsiveness, and a commitment to equity
What You Bring
- A passion for integrated, person-centered care
- Knowledge of medical and behavioral health conditions and the impact of social determinants
- Strong communication, time management, and organizational skills
- Ability to work independently and as part of a team
Education & Experience
- High School Diploma/GED and 4 years of relevant professional experience, or
- Associate’s degree and 2 years of relevant professional experience, or
- Bachelor’s degree in social work, nursing, psychology, sociology, or a related field and 1 year of relevant professional experience
Required Qualifications
- Valid driver’s license and reliable, insured vehicle
- Ability to work in the office during business hours for the first six months, and at least one day per week in-office thereafter
- Proficiency with EHRs, electronic documentation, and email systems
