Attention Social Workers looking to join a dynamic and growing healthcare system - The Sharon Hospital is seeking a Full Time Social Worker to join its Behavioral Health Team.

  • $25,000 SIGN ON BONUS

Responsible for providing social service and discharge planning functions for the inpatient behavioral health unit. Completes psychosocial assessments, contributes to the integrated treatment plan, runs therapeutic groups, coordinates discharge plans. Maintains performance improvement activities within the department and participates in PI activities. Population served based on scope of services in the department.

Essential Responsibilities:

  • Responsible to perform psychosocial assessments within 72 hours of admission.

  • Identifies and addresses psychosocial needs of patient/ family and provides support, counseling and crisis intervention.

  • Ability to assist physicians and communicate, social service, post-hospital care, and discharge planning needs of the patient.

  • Leads insight-oriented, psychoeducational, process, or skills groups.

  • Runs family meetings with both the physician and patient as needed.

  • Provides support with advance directives and assists patient/family with the grieving process and makes referrals to Pastoral Care, as appropriate.

  • Accurately determines and communicates the type of assistance, setting, or resources necessary for the patient/family and provides appropriate resources.

  • Resolves personnel concerns at the departmental level, utilizing the appropriate processes as required.

  • Maintains safety and regulatory requirements, including all state, federal and Joint Commission regulations.

  • Documentation meets current standards and policies.

  • Maintains current knowledge of resources available within the community, maintains supply of resource materials to be distributed to patients when needed.

  • Acts on performance improvement issues identified by behavioral health leadership team.

  • Maintain and Model REACH Values (Respect, Excellence, Accountability, Compassion, Honor).

  • Demonstrates regular, reliable and predictable attendance.

  • Performs other duties as required.

Requirements

  • 2-3 years of experience in allaspects of social service and discharge planning in Hospital setting

  • Master degree SW

  • LMSW license or eligibility (must be obtained within 90 days of hire)

  • LCSW license or eligibility

  • Prior experience in Geriatrics/Emergency settings

  • Bilingual: English/Spanish

Minimum Knowledge, Skills and Abilities Requirements:

• Knowledge of Microsoft Office, including Word, Excel, PowerPoint and Outlook.

• Basic computer knowledge.

• Excellent customer service skills.

• Ability to multitask, make decisions and communicate effectively.

• Able to communicate effectively in English, both verbally and in writing.
