Manager/Peer Support Specialist

Position Description:

The Case Manager or Peer Support Specialist at The Haven will conduct individual case management or Peer Support to assist our residential clients in our all female facility. Our main goals are to start our clients on the road to recovery and lastly provide them with tools for self-sufficiency.

Hours:

Maximum 40 hours/week, Monday – Friday.

Location of Work:
South Jordan, UT

Requirements:

• 18+ years of age
• Certified Peer Support Specialist or Certified Case Manager
• Ability to pass a background check
• CPR & First Aid certification (to take once hired)
• TB test (to take once hired)
• COVID Vaccine
• Valid Drivers License (with less than two infractions in the last 2 years)

Duties:

• Conduct & document needs assessments
• Collaborate on goal setting
• Develop written, individualized case management or support plans with specific outcomes
• Assist with wrap around services
• Conduct Peer Support Groups
• Develop and maintain clear, organized case management documentation
• Maintain compliance with any agency contracts, policies and employment guidelines
• Attend all relevant staff and community meetings
• Other duties as assigned

Job Type: Full-time

Pay: $19.00 per hour

Expected hours: 40 per week
