In 2023, Governor Cox’s One Utah Health Collaborative recognized the IOC has as leader in novel comprehensive care delivery and sustainable financial innovation. The IOC was also featured in Meet Me Where I Am – a short film that premiered in the 2022 Sundance Film Festival – that tells the story of one IOC patient and how this clinic provides unique integrated care.
Working collaboratively with the IOC medical director and interdisciplinary care team, the Clinical Care Coordinator will ensure the successful delivery of exceptional, patient-centered, and integrated care for a targeted population identified as high-risk for poor health outcomes and high utilization of healthcare services. This team works together to deliver high-quality, comprehensive care oriented around treating the bio-psycho-social needs of the IOC patient population.

At minimum, the ideal candidate will have a working knowledge of trauma and substance use disorders, insurance enrollment and claims processes, strong connections with community and social programs, and demonstrated success in coordinating patient care within a multidisciplinary clinical setting. Case management experience is preferred.
